Thank you for ordering from ISA Data please take a minute to review the policies and instructions for ordering outlined below and then click “I agree” to continue
Things you will need to know before you order:
  1. Your survey type
  2. The schools that will be participating and their respective districts
  3. Enrollment, or how many surveys you want per each school
  4. A estimated date you plan to survey
It’s important to remember that your goal is to get as close to the actual number of students that will take the survey as possible. We charge by survey as opposed to by report to keep our product equitable for small and large organizations. Some guidelines for how to do this are as follows:
  1. Our minimum survey order is 200 surveys.
  2. We expect a return rate of 70% +- 10% from most schools that participate. This number can change if your state has active consent laws. Please let us know if that is the case before you order.
    • Ex. for a school with 1000 students in grades 6-12 we expect 600-800 respondents
  3. If you over-order surveys, our policy is as follows:
    • ISA Data invoices based on the amount of survey instruments ordered. If you need more paper surveys, we will ship extras and bill you for the extra order plus shipping. If you submit more online surveys than originally ordered, we will bill you for the extra online surveys submitted.
    • If you do not use your full order of survey instruments, the surveys can be used to gather data for another survey cycle, as long as that survey is administered within 2 years of the original purchase date.
    • We do not offer refunds or credits for unused surveys (paper or online).
    • If you have leftover surveys and do not place an order for more surveys in a new survey year there is an administrative fee equal to the cost of 200 surveys that will be applied to the order.
  4. Paper survey use:
    • Do not discard leftover paper survey instruments.
    • If leftover paper surveys will be used from the prior two-year period, please call before using them to make sure those forms are the most current. If they are not, we will replace those forms with the most current survey version.
  5. Online survey use:
    • We ask that you provide a date for the survey so that we can pre-warm the servers to handle a heavy load of users all at once. If we do not know the survey date in advance there is a chance that the system will need to scale automatically and that error messages will be displayed as the system adds capacity. Please wait five (5) minutes before trying to log students in again.
    • Any student who begins a survey and makes it past the first page, whether the survey results in a valid survey or not counts as a survey that has been used. A valid survey is a survey that ends up as a part of the final data set and an invalid survey is one that is removed from the final data set for any reason.
We make all efforts possible to maintain our systems. In the event of a catastrophic failure or act of god that causes the breakdown of national or global systems, surveys may need to be re-taken by the student body. In the extremely rare event ISA Data systems are at fault, we will notify you, our customer of the mistake and either re-run the survey, or offer a credit based on the the amount of product that was faulty, or, at our discretion, offer a refund for the cost of the surveys that were faulty.
