
ISA DATA Survey for grades 6-12 (Beta Testing)
This survey will replace the Pride Survey family of surveys in 2026-2027 with updates throughout. These include: A new demographic category of Middle Eastern or North African; A new demographic category for sexual preference; Updated substances including additional classifications of THC and CBD including delta 8, 9, and 10 substances as well as THCa; New questions about convenience store substances that are currently available; Open ended vape questions to determine locally specific brands that are popular with youth in your area; Updates throughout to examples to bring them in line with current language and trending substances; Added new optional DFC measures on Heroin and Methamphetamine; Updated reports with new easier to read and understand graphics. ISA Condensed DATA Survey for grades 6-12 (Beta Testing)
This survey is a condensed version of the ISA DATA Survey and is intended to help coalitions solve a few potential issues. This survey will exclude items like sexual preference for schools who are facing pushback from parents and political organizations about sensitive items. This survey will also exclude questions around family life, school life, and health making it a targeted survey about substance use and abuse. The rationale being that many schools and states already have surveys in place that collect much of the data and are looking for something community coalition specific to assist with school buy-in. Beta testing of this product will be available starting in 2025, if you would like to help test the new survey with your school for potentially deep discounts please contact us at (800)279-6361. PS Plus Survey for grades 6-12
Introduced in 1982, the Pride Student Survey for Grade 6-12 has been employed in thousands of schools across the country with millions of students participating. Much more than just a substance abuse survey for students, this questionnaire also gathers school climate data (on bullying, harassment, etc.) as well as 50 risk and protective factor items. The Pride Surveys Plus is a child of the original Pride Survey which added 2 categories to the existing core measures including prescription opioids as well as e-cigarettes as well as questions about cell phones and questions about where and how students obtain products that contain nicotine. This survey will be phased out over the next few years for the ISA DATA and the ISA CDATA Surveys are finalized and tested.

DFC Core Measures Supplimental Survey
The Drug-Free Community survey module is designed to be used in conjunction with another survey instrument, such as the Youth Risk and Behavior Survey. We recommend that the Drug-Free Community survey not be used alone, as the limited data gathered may not be enough to support a prevention logic model. The single-page, one-sided survey should be used when you are already collecting student data, but the instrument you are using does not meet your grant’s data collection requirements.
In addition to the Drug-Free Community survey, all of our standard surveys for middle and high school students collect the four core measures required by SAMHSA and have been pre-approved by the DFC evaluation team.

PS Survey for grades 4-6
Developed in 1988, Pride’s Student Surveys for Grades 4-6 collect data from intermediate grade students as they begin to develop attitudes and behaviors about alcohol, tobacco and other drug use, as well as violence and bullying. It is the most widely used elementary questionnaire in the nation, with over 1.3 million students having been surveyed to date. In addition to serving as a reliable substance abuse survey for students, these questionnaires also gather information regarding home and family life, how students perceive violent behaviors, and what they do in their free time. This survey is slated to be updated for 2027 as the ISA DATA survey for grades 4-6.

Communities that Care Youth Survey
The Communities That Care (CTC) Youth Survey belongs to the Center for Substance Abuse Prevention in the office of the United States Government’s Substance Abuse and Mental Health Services Administration (SAMHSA). This youth community survey is a questionnaire for use by community coalitions that uses the risk and protective factor model approach to assess youth problem behaviors, such as violence, delinquency, school dropout and substance abuse. The CTC youth risk survey offered by Pride Surveys is an exact replica of the original, with the addition of SAMHSA’s new Core Measures, required by the Drug-Free Communities Grant effective February 2013.

Risk and Protective Factor Survey
With the expert assistance of Dr. Jack Pollard of the University of Washington, Pride Surveys developed its Risk and Protective Factor (RPF) student perception survey, a hybrid version of the Communities That Care (CTC) Youth Survey and the Pride Questionnaire for Grades 6 to 12. Appropriate for students in grade 6 through 12, the RPF survey report contains scale scores, familiar to previous users of the CTC Youth Survey, as well as percentage responses for each risk and protective factor item.

Social Emotional & Bullying Behavior Survey
With SEBBS, schools can discover underlying causes for unwanted conditions such as increased absenteeism, lower academic achievement and increased substance abuse. Many schools can use the results to align with the school’s improvement plan and anti-bullying policies at both the school and state levels. Also, schools can use the data to apply for funding for anti-bullying programs as well as add value by defining and promoting the school’s positive learning environment. Some schools choose SEBBS as a pre- and post-test for bullying prevention programming, e.g., the Second Step program.

Learning Environment Survey
The ISA Data Learning Environment Survey is a student evaluation tool for grades 6-12. because of its detailed reports and examination of current issues in education, as well as its inclusion of National Outcomes Measurement System items required as benchmarks for many grant awards it remains an excellent tool for evaluating school climate. This comprehensive school climate survey measures many facets of the learning environment. With this tool, students, teachers and administrators, can work as a team to address the school’s weaknesses while building upon its strengths.
